    What happens to student engagement and understanding of chemistry when cross-curricular activities and assignments are used in an honors chemistry class?

    The focus of this action research study was to determine if cross-curricular learning strategies could be used in a chemistry class to increase both student engagement and understanding of chemistry content. Two strategies were implemented in an honors chemistry course of sophomores and juniors. Participants were chosen on a voluntary basis, those students who chose not to participate still needed to complete the activities, however, their grades and responses were not included in the data. Participating students were required to complete a consent form and obtain parental permission. Students completed a creative writing activity and a kinesthetic activity in which they demonstrated their understanding of the content. The students showed an increase in engagement and understanding of chemical concepts that were a part of this research study. Thus, the conclusion of this research study suggests that implementing crosscurricular learning strategies increases student engagement and understanding of chemistry

    Staff engagement for practice change in long-term care: evaluating the Feasible and Sustainable Culture Change Initiative (FASCCI) model

    Context: Interventions aimed at increasing the provision of person-centred care in long-term care (LTC) homes, that do not address contextual and system issues, most often fail. Promoting positive change in LTC homes requires requires a multilevel, systems approach. Objectives: Evaluate the effectiveness of the Feasible and Sustainable Culture Change Initiative (FASCCI) model for improving the provision of person-centred mealtime practices in a LTC home. Methods: A single-group, time series design was used to assess the impact of the FASCCI model for change on outcome measures across four time periods (pre-intervention, 2-month, 4-month and 6-month follow-up). Differences in scores from baseline were assessed utilizing Wilcoxon signed-rank tests. Interviews (n = 21) were also conducted to examine treatment fidelity and to ascertain the study participants’ perceptions of the process for making improvements using the FASCCI model. Findings: We observed increases in care staff’s capacity to consistently provide relational and person-centred care during mealtimes. Mealtime environment scores started increasing immediately following the intervention, with statistically significant improvements in all mealtime environment scales by six-months, including: the physical environment (W = 55.00, p = 0.008); social environment (W = 55.00, p = 0.008); relationship-centred care (W = 45.00, p = 0.014); and overall quality of dining environment (W = 55.00, p = 0.010). Analysis of data from qualitative interviews demonstrated that use of the FASCCI model resulted in improved team leadership, communication, and collaborative decision-making. Limitations: Generalizability is limited due to the small sample size and use of convenience sampling methods. Implications: Outcomes indicate that the FASCCI model seems promising in its ability to improve PCC mealtime practices in LTC homes and is worthy of a larger scale study. The results further demonstrate the value of supportive team environments in quality dementia care

    Angiogenic Biomarkers for Risk Stratification in Women with Suspected Preeclampsia

    This poster presents the results of a single-center prospective cohort study of 315 pregnant women who presented to George Washington University Hospital Labor and Delivery service with a clinical suspicion of preeclampsia between February 2007 and November 2011. Informed consent was obtained. Baseline demographic information and medical history was collected on each patient including maternal age, ethnicity, body mass index, tobacco and other substance use, gestational age, medical problems and obstetric history. Serum samples were obtained from each enrolled subject within 24 hours of admission, and sFlt1, PlGF and sEng ELISA assays were performed in duplicate by a blinded laboratory technician at the University of Massachusetts

    The role of trained champions in sustaining and spreading nutrition care improvements in hospital: qualitative interviews following an implementation study.

    BACKGROUND: Many patients are already malnourished when admitted to hospital. Barriers and facilitators to nutrition care in hospital have been identified and successful interventions developed; however, few studies have explored how to sustain and spread improvements. The More-2-Eat phase 1 study involved five hospitals across Canada implementing nutrition care improvements, while phase 2 implemented a scalable model using trained champions, audit and feedback, a community of practice with external mentorship and an implementation toolkit in 10 hospitals (four continuing from phase 1). Process measures showed that screening and assessment from phase 1 were sustained for at least 4 years. The objective of this study was to help explain how these nutrition care improvements were sustained and spread by understanding the role of the trained champions, and to confirm and expand on themes identified in phase 1. METHODS: Semistructured telephone interviews were conducted with champions from each phase 2 hospital and recordings transcribed verbatim. To explore the champion role, transcripts were deductively coded to the 3C model of Concept, Competence and Capacity. Phase 2 transcripts were also deductively coded to themes identified in phase 1 interviews and focus groups. RESULTS: Ten interviews (n=14 champions) were conducted. To sustain and spread nutrition care improvements, champions needed to understand the Concepts of change management, implementation, adaptation, sustainability and spread in order to embed changes into routine practice. Champions also needed the Competence, including the skills to identify, support and empower new champions, thus sharing the responsibility. Capacity, including time, resources and leadership support, was the most important facilitator for staying engaged, and the most challenging. All themes identified in qualitative interviews in phase 1 were applicable 4 years later and were mentioned by new phase 2 hospitals. There was increased emphasis on audit and feedback, and the need for standardisation to support embedding into current practice. CONCLUSION: Trained local champions were required for implementation. By understanding key concepts, with appropriate and evolving competence and capacity, champions supported sustainability and spread of nutrition care improvements. Understanding the role of champions in supporting implementation, spread and sustainability of nutrition care improvements can help other hospitals when planning for and implementing these improvements.     Cross-Sectional Nutrition Profile of Palliative Home Care Clients in Ontario and Performance of the interRAI Palliative Care Nutrition Clinical Assessment Protocol

    Background: The nutrition profile of palliative home care clients is unknown. This study describes this group and their nutrition issues and evaluates the performance of the interRAI nutrition Clinical Assessment Protocol (CAP). Methods: This was a cross-sectional secondary analysis using Ontario interRAI Palliative Care (interRAI PC) Assessment data. The sample represents 74,963 unique Ontario home care clients assessed between 2011 and 2018. Frequencies and standardized differences (stdiffs) of nutrition characteristics were presented for cancer (n = 62,394) and noncancer (n = 12,569) diagnostic subgroups. Rates of triggering the nutrition CAP were presented by nutrition issue to evaluate its performance. Results: Of this sample, 16.7% were ≥85 years of age, 52.6% had a prognosis between 6 weeks and 6 months, and 41.4% required assistance with eating. The prevalence was higher among those with nervous/mental/behavioral disorders (72.6%) compared with those with cancer (37.6%; stdiff = 0.75). However, most nutrition issues experienced were similar (stdiff \u3c 0.20) across diagnostic groups. Of the entire sample, 21% triggered the nutrition CAP, indicating a need for further evaluation or intervention. Yet, 73.4% of those who experienced dry mouth, 71.8% of those who required assistance with eating, and 68.4% of those who received a nutrition consult within the last 3 days did not trigger the nutrition CAP. Conclusions: Nutrition issues are prevalent in palliative home care clients, regardless of diagnosis; yet the nutrition CAP identified a small fraction of this group. There is a need to focus research and care guidelines toward life-limiting illnesses beyond cancer and address nutrition-related issues in this population

    Strategies to support engagement and continuity of activity during mealtimes for families living with dementia; a qualitative study

A longitudinal qualitative study provided understanding of the meaning of mealtimes for persons with dementia and their family care partners. Strategies were specifically described by families to support meaningful mealtimes. The purpose of this manuscript is to describe the strategies devised and used by these families living with dementia. Methods A longitudinal qualitative study was undertaken to explore the meaning and experience of mealtimes for families living with dementia over a three-year period. 27 families [older person with dementia and at least one family care partner] were originally recruited from the community of South-Western Ontario. Individual and dyad interviews were conducted each year. Digitally recorded transcripts were analyzed using grounded theory methodology. Strategies were identified and categorized. Results Strategies to support quality mealtimes were devised by families as they adapted to their evolving lives. General strategies such as living in the moment, as well as strategies specific to maintaining social engagement and continuity of mealtime activities were reported. Conclusions In addition to nutritional benefit, family mealtimes provide important opportunities for persons with dementia and their family care partners to socially engage and continue meaningful roles. Strategies identified by participants provide a basis for further education and support to families living with dementia.Research funding support was provided by the Alzheimer Society of Canada and the Social Sciences and Humanities Research Council

    The Aquarius Co-Moving Group is Not a Disrupted Classical Globular Cluster

    We present a detailed analysis of high-resolution, high S/N spectra for 5 Aquarius stream stars observed with the MIKE spectrograph on the Magellan Clay telescope. Our sample represents one third of the 15 known members in the stream. We find the stream is not mono-metallic: the metallicity ranges from [Fe/H] = -0.63 to -1.58. No anti-correlation in Na-O abundances is present, and we find a strong positive Mg-Al relationship, similar to that observed in the thick disk. We find no evidence that the stream is a result of a disrupted classical globular cluster, contrary to a previously published claim. High [(Na, Ni, alpha)/Fe] and low [Ba/Y] abundance ratios in the stream suggests it is not a tidal tail from a disrupted dwarf galaxy, either. The stream is chemically indistinguishable from Milky Way field stars with the exception of one candidate, C222531-145437. From its position, velocity, and detailed chemical abundances, C222531-145437 is likely a star that was tidally disrupted from omega-Centauri. We propose the Aquarius stream is Galactic in origin, and could be the result from a disk-satellite perturbation in the Milky Way thick disk on the order of a few Gyr ago: derived orbits, UVW velocities, and angular momenta of the Aquarius members offer qualitative support for our hypothesis. Assuming C222531-145437 is a tidally disrupted member of omega-Centauri, this system is the most likely disk perturber. In the absence of compelling chemical and/or dynamical evidence that the Aquarius stream is the tidal tail of a disrupted satellite, we advocate the "Aquarius group" as a more appropriate description. Like the Canis Major over-density, as well as the Hercules and Monoceros groups, the Aquarius group joins the list of kinematically-identified substructures that are not actually accreted material: they are simply part of the rich complexity of the Milky Way structure.Comment: Accepted to MNRAS.
